The San Diego American Indian Health Center (SDAIHC) is a patient-centered health home that provides comprehensive medical, dental, behavioral health, and community wellness services that are available to all San Diegans. Founded in May 1979, we honor the wisdom of community leaders who believed urban American Indians living in our city should have community-based health care rooted in traditional cultural values. The vision of our founders lives on after nearly forty years as we continue to grow and provide high quality health care services to a diverse community of all ages and walks of life.

SDAIHC is accredited by the National Committee for Quality Assurance as a Patient Centered Medical Home, a recognition that demonstrates our commitment to providing evidence-based whole-person care.

The Patient Center Medical Home is a healthcare delivery model that ensures each patient receives the best possible care: when they need it and in a manner they can understand. A team of providers meets the physical and mental needs of the patient and coordinates care across multiple settings, with special consideration of the individual’s culture, values, and preferences. A PCMH works in partnership with patients and their families—each is an important member of the care team—so they are empowered to make healthy lifestyle choices.

As a PCMH, we continuously evaluate our services and the health outcomes of our patients to guide quality improvement. We also encourage patients to gain an understanding of their role in a PCMH by reading the Patient Rights and Responsibilities form and the Notice of Privacy handout, both of which are available on our website and in our health center.
